Here's an example sentence: "I had had a fever until yesterday."

It's just a coincidence that "had had" are together. It's the past perfect tense (http://www.englishpage.com/verbpage/pastperfect.html), in which "had" is combined with the past participle of the verb "have," which happens to be "had."

"Ego" is singular: Jo's mean comments hurt my ego. "Egos" is plural: Jo's means comments hurt our egos.
